Present Participants - There is an “Open Enrollment” each year during the month of November for a January 1st effective date. All changes for enrolled employees must be made during that time unless there is a “Life Style Change” (birth, marriage, etc). Those changes must reach our office within 31 days of the “event.” Changes received after 31 days will not be accepted until the next “Open Enrollment.” Eligibility requires a member to be in current good standing with the Association. An eligible employee is an active full-time employee working an average of thirty (30) hours per week or 1560 hours per year. There is a minimum one-month waiting period from the date of hire for any coverage. If the plan is “Contributory” for a company, 50%* of eligible employees must be enrolled for coverage. If the plan is “Non-contributory” for a company, all eligible employees must be enrolled*. All eligible employees must be accounted for. Those declining coverage must complete and sign a “Declination” form at the time of eligibility. Any company leaving the plan for other coverage may return after a a period of one year. New groups: Final Tier will be determined upon application and medical underwriting.
